Stellar Care and Services first began offering Supported Community Connections Adult Day Program services in 2009 and has expanded to offer a number of other services including Residential and Supported Employment Services. We serve people with disabilities from two locations in the State of Colorado. See our locations for more information regarding what services are provided at each location.
Stellar Care and Services serves close to 300 individuals with intellectual and developmental disabilities throughout our various programs.

This depends on our Stellar Care and Services location.

Our Colorado Springs Office provides Adult Day Program Services, Residential Services, Supported Living Services/Community First Choice Services, and Supported Employment Services.

Our Denver-Metro Area Office provides Adult Day Program Services and Residential Services.

We serve adults over the age of 18.

We are able to serve any individual in the State of Colorado. However, our Colorado Springs Office generally covers the southern region; including Pueblo, Teller, and surrounding areas. While our Denver-Metro Area Office located in Westminster covers the northern region; including Boulder, Fort Collins, and surrounding areas. To see if you are able to access services from one of our offices, contact the office that is closest to your home.

To see if you are eligible for services, contact your local Community Center Board. A list of them can be found through The Colorado Department of Health and Environment. If you are not eligible for waiver services, you may consider privately paying for services. Visit www.colorado.gov/cdphe for more information.

Residential Services FAQs

A Host Home is a private residence contracted with a trained provider to support adults with intellectual and developmental disabilities. The goal is to create a safe, respectful, and nurturing environment where individuals can live with dignity, build independence, and continue to grow.

Stellar Care staff work closely with providers to ensure services are person-centered and that each individual’s unique needs are met. Host Home providers receive a daily rate based on the level of support required for the person in their care.

The main role of a Host Home provider is to offer a safe and stable home where individuals can make choices, set and reach personal goals, and develop greater independence. Host Homes provide more one-on-one support, allowing for truly individualized care that helps each person thrive.
A Family Caregiver is considered a Personal Care Alternative. While they may be related to the individual receiving services, Stellar Care provides the same training, support, and guidance to Family Caregivers as we do to unrelated Host Home providers.

All providers must complete state-required training, and Stellar Care also offers additional, personalized training to ensure the best support for the individuals we serve. Required trainings include:

  • Orientation & Expectations
  • First Aid/CPR
  • QMAP
  • Mandt
  • MANE/IR
  • SOMB

Stellar Care also offers a comprehensive Apartment Program for individuals who are ready to live more independently. Supports are tailored to each person’s strengths and goals and may include:

  • Assistance with medical visits and medication management
  • Goal setting and life skills development
  • Grocery assistance, meal planning, and meal preparation
  • Community integration opportunities
  • Support for personal autonomy
  • Additional amenities and supports are available on a case-by-case basis to ensure every individual has what they need to succeed.
The Apartment Program Services provides services to Adults with IDD who are capable of living in their own homes receive guidance to develop the everyday life skills needed to succeed and achieve their individualized goals. The Apartment Program services are funded through the HCBS-DD Waiver, ensuring that individuals receive the support they need to live independently, confidently, and safely.

Adult Day Program Services FAQs

The SCC Adult Day Program is a group-formatted, community-based program designed to support adults with intellectual and developmental disabilities in building skills, independence, and meaningful connections.

At Stellar Care, we believe that smaller staff-to-individual ratios are key to personal growth. We maintain a maximum ratio of 1 staff to 4 individuals, and some participants receive 1:1 support based on their needs.

Our services are person-centered—we prioritize listening to each individual, identifying activities they enjoy, and helping them engage with their communities in ways that are meaningful and empowering. Participants work toward their community integration goals in a supportive, compassionate environment.

All of our Day Programs take place in the community, and door-to-door transportation is provided for convenience and accessibility.

Stellar Care’s Day Program operates Monday through Friday with the following schedule:

  • Pick-ups: 8:00 AM – 9:00 AM
  • Program Hours: 9:00 AM – 3:00 PM
  • Drop-offs: 3:00 PM – 4:00 PM

Services are provided according to each person’s Individual Service Plan (ISP). We also offer occasional evening and weekend activities, such as dances or special events, to support social engagement and community involvement.

Our Specialized Habilitation Program is dedicated to empowering individuals with the skills and confidence needed for greater independence and community engagement. Through personalized support and structured learning, participants build essential life skills in areas such as communication, self-care, social interaction, and daily living.
We focus on each individual’s strengths and goals, creating a supportive environment where progress is both measurable and meaningful. Whether you’re seeking to enhance autonomy or improve quality of life, our program offers a compassionate and goal-driven approach to personal development.

Supported Living/Community First Choice Services FAQs

Supported Living Services (SLS) and Community First Choice Services are used to supplement already available supports for adults who either can live independently with limited supports, or if they need extensive support, are principally supported from other sources, such as the family. SLS provides a variety of services, such as personal care (medical appointments, grocery shopping, etc.) or homemaking needs, mentoring, accessing his or her community, help with decision-making, professional therapies, and transportation. Supported Living Services are not intended to meet all needs. We also offer respite services through SLS, which can range from hourly mentor provided services to the utilization of our host homes for overnights.

Supported Living/Supported Employment FAQs First Choice Services FAQs

Supported Employment Services help individuals with disabilities find and maintain meaningful jobs in their communities. Through personalized guidance, job coaching, and partnerships with businesses, these services support both employees and employers to create inclusive, successful workplaces. To receive services, individuals must have a Department of Vocational Rehabilitation (DVR) waiver.

A Job Coach is a trained professional who provides personalized, on-the-job support to help individuals with disabilities succeed at work. They assist with learning job tasks, building workplace skills, communicating with employers, and adjusting to new environments. Job Coaches also support businesses by helping ensure clear communication, smooth onboarding, and ongoing success for both the employee and the employer.
